Appointments

To schedule an appointment, patients with insurance must have a physical therapy prescription from a referring physicians. Please bring the prescription and an insurance card. Patients should bring claim info if they are being treated as the result of a work injury or motor vehicle injury.

Self-pay patients who do not have insurance are not required to have a physical therapy prescription from a physician when making an appointment.
