Making an Appointment

To schedule an appointment, patients with insurance must have a physical therapy prescription from a referring physicians. Please bring the prescription and an insurance card. Patients should bring claim info if they are being treated as the result of a work injury or motor vehicle injury.

Self-pay patients who do not have insurance are not required to have a physical therapy prescription from a physician when making an appointment.

We are open Monday through Friday from 7 a.m. to 6 p.m. Please call us at 360.578.1188 to schedule your appointment.
